Use of S-100B, NSE, CRP and ESR to predict neurological outcomes in patients with return of spontaneous circulation and treated with hypothermia

Background

With the introduction of therapeutic hypothermia (TH), the prediction of neurological outcomes in cardiac arrest (CA) survivors is challenging. Early, accurate determination of prognosis by emergency physicians is important to avoid unnecessarily prolonged critical care with a likely poor neurological outcome.

Methods

This prospective observational study included patients with non-traumatic CA and return of spontaneous circulation (ROSC) b…

An infraumbilical lump in a child

Clinical introduction

A 13-month-old boy was brought to the emergency department by his mother with a 2-day history of a painless, firm lump located in the midline of his abdomen, just inferior to the umbilicus. The lump was skin coloured, not hyperaemic and non-reducible.

The patient was afebrile without any change in bowel habits or any preceding illness. Apart from a history of being born at 32 weeks’ gestational age and spending some time under obs…

Ovarian cyst- பெண்கள் இனப் பெருக்க மண்டல பிரச்சனை

Ovarian cyst
Ovarian cysts are fluid-filled sacs or pockets within or on the surface of an ovary.
Types:
Functional cysts
• Follicular cyst. A follicular cyst begins when something goes wrong and the follicle doesn’t rupture or release its egg. Instead it grows and turns into a cyst.
• Corpus luteum cyst. When a follicle releases its egg, the ruptured follicle begins producing large quantities of estrogen and progesterone for conception. This follicle is now called the corpus luteum. Sometimes, however, the escape opening of the egg seals off and fluid accumulates inside the follicle, causing the corpus luteum to expand into a cyst.
Other cysts
Dermoid cysts. These cysts may contain tissue, such as hair, skin or teeth, because they form from cells that produce human eggs..
• Cystadenomas. These cysts develop from ovarian tissue and may be filled with a watery liquid or a mucous material.
• Endometriomas. These cysts develop as a result of endometriosis, a condition in which uterine endometrial cells grow outside your uterus. Signs and symtoms
• abdominal bloating or swelling
• painful bowel movements
• pelvic pain before or during the menstrual cycle
• painful intercourse
• pain in the lower back or thighs
• breast tenderness
• nausea and vomiting
• changes in menstrual cycle
• ongoing pelvic pain
• loss of appetite
• unexplained weight loss
• abdominal fullness
Complications
• Ovarian torsion. Cysts that become large may cause the ovary to move out of its usual position in the pelvis. This increases the chance of painful twisting of ovary, called ovarian torsion.
• Rupture. A cyst that ruptures may cause severe pain and lead to internal bleeding.
• severe or sharp pelvic pain
• fever
• faintness or dizziness
• rapid breathing
• Diagnosis
• Ultrasound: an imaging device used to visualize the ovary
• CT scan: a body imaging device used to create cross-sectional images of internal organs
• MRI: a test that uses magnetic fields to produce in-depth images of internal organs
• pregnancy test: to determine whether or not pregnant
• hormone level test: to check for hormone-related issues, such as too much estrogen or progesterone
• CA 125 Blood test: to screen for ovarian cancer
• Laparoscopy

Treatment
pain relievers, including acetaminophen, nonsteroidal anti-inflammatory drugs ,or narcotic pain medicine.
• A warm bath, or heating pad, or hot water bottle applied to the lower abdomen near the ovaries can relax tense muscles and relieve cramping,
• chamomile herbal tea can reduce ovarian cyst pain and soothe tense muscles, urinating as soon as the urge presents itself.
• Avoiding constipation, which does not cause ovarian cysts but may further increase pelvic discomfort.
• In diet, eliminating caffeine and alcohol, reducing sugars, increasing foods rich in vitamin A and carotenoids (e.g., carrots, tomatoes, and salad greens) and B vitamins Combined methods of hormonal contraception such as the combined oral contraceptive pill
Aloevera, amla, green vegetables, garlic, bread, and oats helps to prevent ovarian cyst. Cake and frozen items to be avoided. Stress, sleeplessness, thyroid problem to be corrected.

Laparotomy
A hysterectomy to remove ovaries and uterus.

Vigils were staged across Germany to commemorate Tugce Albayrak, a 22-year-old woman who was killed after stopping a group of men from sexually harassing two teenage girls at a fast food restaurant in mid-November.

According to reports, Tugce Albayrak heard someone screaming for help at a McDonald’s in Offenbach, a city near Frankfurt. She headed to the toilet and confronted a group of men who were harassing two teenage girls.

Surveillance footage obtained by German newspaper Bild shows that Albayrak’s attacker, identified as Senal M, went to the car park.

In an agitated state, Senal M left the car park and returned shortly, where he beat Albayrak’s head with a bat. His friend failed to stop him. The impact was so powerful that the young woman fell on the ground after the strike.

Albayrak’s parents agreed to turn off life support of their daughter last Friday, which was her birthday.

Albayrak’s killer, reportedly an 18-year-old Serbian man, is now in police custody. More than 100,000 people have signed a petition calling for the German government to award a posthumous award to Albayrak, who was of Turkish descent.

So I’m gonna demonstrate how to form roll for lower back pain. Unfortunately many of us suffer from lower back pain. So the first thing you wanna do is get the roller lie it down like this. You’re gonna apply pressure to one side of your lower back while supporting yourself with your elbow.

You wanna be careful here. A lot of us have herniated discs, bulging discs so you really want to take your time. You could raise your hips up just like this, that will take some of the tension and pressure off your lower back.

Explore the area, see whereabouts your tender. Do this for about two minutes. Focus on the points that you are feeling the most tension and pain.

You’re gonna switch sides. Switching arms, rolling toward the very bottom of your lower back to the middle. When we’re experiencing lower back pain most of the time it’s not coming from our back. I tell my clients to do like a flush of the lower back if you are experiencing pain there but definitely you wanna hit your glutes.

A lot of the time our hips and our glutes are very tight and that’s what’s causing the lower back pain. So you wanna focus on, you could cross your ankle, one ankle across your bottom thigh and just focus on one side of your glute, glute med, glute max, toward the middle.

Doing this for about two minutes, switching legs. The reason why I’m crossing is because I’m also gonna get into the piriformis which is a smaller muscle that actually tightens up just from sitting so that’s also getting deeper into your glute medius in the piriformis.

Always making sure alignment is priority so your hands should be directly under your shoulder. You also wanna make sure you’re rolling your hips, your hip flexors if you’re experiencing lower back pain because again, a lot of the time when our lower back is acting up it’s not coming from the culprit, it’s not coming from something in your lower back it’s usually something around it.

So let’s roll the hips. You’re gonna get back on your hands and knees and hit the very top of your thigh bone area. It could almost feel like a dull pain. That’s how I would describe what the hip flexor feels like when you roll it.

One to two minutes on side and then one to two minutes on the other. And that’s how you form roll to improve lower back pain.

Are there too few women presenting at emergency medicine conferences?

Introduction

There is a perception that women are under-represented as speakers at emergency medicine (EM) conferences. We aimed to evaluate the ratio of male to female speakers and the proportion of presenting time by gender at major international EM conferences.

Methods

Conference programmes of the major English-speaking EM conferences occurring from 2014 to 2015 were obtained. The number of presentations, the gender of the speaker and the duration…

http://www.lifeplusvitamins.com/bodysmart-program.htm – This program was developed by a medical doctor in Germany using the LOGI method, which is based on eating low-glycemic foods. In large part what makes the program work so well is it jump starts your metabolism. One of the great side benefits of this program besides losing weight is it will greatly increase hour energy levels. You will be amazed at how great you feel.

On both days geared toward metabolism readjustment you may consume up to 5 Protein/Nutrition Shakes. You should not consume solid food during these two days, but drink a large amount of liquids: a minimum of 2 to 3 liters of water, tea, coffee, broth.

You should not let more than 3 hours pass between consumption of the shakes, otherwise you run the risk of consuming too few calories and your metabolism will switch to starvation mode instead of fat-burning and weight-reduction mode.

Mix 1 scoop (20 g) of Triple Protein Shake in at least 500 — 700 ml of liquid (water or reduced-fat milk). Add 1-2 tea spoons of flax oil or coconut oil. Take 2 tablets BodySmart Gold (Women or Men) and 2 tablets EnerXan with your first shake. People with high blood pressure should start out with 1 tablet of EnerXan

http://www.lifeplusvitamins.com/supershake.html
http://www.lifeplusvitamins.com/enerxan.html
http://www.lifeplusvitamins.com/mens-gold.htm
http://www.lifeplusvitamins.com/womens-gold.htm

Once you reach your target weight, you can switch back to 3 normal meals per day. A diet according to the Logi method is highly suitable: Carb-reduced, modified fat intake, optimal protein intake. However, it is advisable to occasionally replace a meal with a protein shake. The most suitable meal is dinner as it will boost fat burning during the night.

You will only continue to lose a minimal amount of weight during the Stabilization Phase as the goal during this phase is stabilizing your target weight.

With the diet according the Logi method you are able to follow a balanced diet, keep your weight in the “ideal area,” and remain effective. During times of increased nutrient requirements (exercise, stress, pregnancy, smoking), the nutritional balance can be easily maintained with concentrated micro nutrients.

When will we have enough women speakers in emergency medicine?

In an observational study published in EMJ, Carley et al1 asked “Are there too few women presenting at emergency medicine conferences?” The authors analysed the apparent gender of speakers at eight major emergency medicine (EM) conferences from 2014 to 2015. They found that women presenters gave 29.9% of the talks occupying 27.6% of the total speaking time.
